US passport name change endorsement for OCI

we recently got US citizenship and passports. at the time of naturalization test, USCIS officer talked to my wife if she wants name change. so my wife changed her last name to be mine. now when i want to apply for OCI, they are asking for endorsement during naturalization or court order(??) since her name in indian passport is different from US passport. otherwise OCI app would be rejected. is that true? if i really need that endorsement, how do i get it now? any way i get it without making a trip to USCIS?

does anyone know how to get the endorsement? Can i go back to USCIS and request them? is there any webpage or something telling what is the procedure, application form, fee etc?
