Assuming you are filing under the "5-year" rule, you can submit within the 90-days before you actually reach 5 years from the issue date of your first Green Card (start of legal residency).
You should submit the following:
1. Completed and signed N-400 (new version with 21 pages)
2 Check for $680 to "U.S. Department of Homeland Security
3. Two passport-style photos of you
4. Copies of front and back of your Green Card
5. Copies of all divorce documents from all prior marriages
6. Copy of most recent Federal income tax return or transcript...as proof that you are meeting your tax-filing responsibilities
