Birth Cirtificate query

Hi,
My last Name is different in insurance card and passport....now I am going to deliver my baby in 2 weeks...and I want my name should appear in my babys birth cirtificate same as my passport name not as insurance card.....I dont know whether it is possible or not...Suggestion needed....

I believe it is possible and even required in some states to show your ID before you can get your child's birth certificate. I guess because of the undocumented aliens they made the procedure of getting birth certificate pretty long in some states. I had to go to the local health dept, fill in some form, mail in the fee, show ID, then got my son's birth certificate in a mail later. So your name will be spelled like it is in your Passport, I believe
But anyway don't worry about that kind of stuff too much and have a safe delivery
