Change of Name in Renunciation form

i have a question about change of name option in renunciation form. my wife added our surname to her passport after marriage, that passport is now expired her current Indian passport has her full name with surname added, as she is a US citizen now and wants to renunciate her Indian passport my question is what should she answer for change of name column.
Do we need to mention the change of name even if her current Indian passport name matches US passport?
if needed to mention change of name what documents are needed to show proof ,she only has her previous Indian passport where the name change was entered.
