N400 Part 13-2 How to fill correctly

Hello I’m new to this forum, I just joined because I need help preparing my spouse’s N400 application regarding one question. It’s part 13 question 2 - Applicant’s Statement Regarding the Preparer.

im preparing their application so I checked the box that says: At my request, the preparer named in Part 15., [_________], prepared this application for me based only upon information I provided or authorized.

How should I fill out the box? I really appreciate the help. Thank you.

i read it as you would put the preparer's name in that box (in this case i assume your name!)
