    Dear Sir,

    I am a Merchant Marine Officer and am in the process for applying for a fresh C1/D visa using Forms 156 and 157. In the event that the space available in any of the boxes on the forms is inadequate, can the applicant provide the information on additional sheets of paper?

    For example in Form 156 - Box 29 - Have you ever been to the U.S. ?
    A merchant marine seaman may have been to the U.S. numerous times and the space is not adequate to fill in all the information.

    Similarly in Form 157 - Box 9 - List all countries you have entered in the last ten years (Give year of each visit) This may turn out to be an enormous list for a merchant marine seaman and the provided space in the form is definitely inadequate.

    I would be most grateful if you can provide your guidance on this matter.
