J-1 Visa Waiver for Pakistan - Help Needed

Hi -

I'm a J-1 student from Pakistan. Since my program is funded by US State Department, I'm subjected to fulfill the 2 years home residency requirement. I'm planning to get the waiver on that requirement through No Objection Statement.

I tried finding the NO Statement Form on Pakistan Embassy website but couldn't see any. Can anyone please tell me where to get that form? Also, please share your experience if you were a J-1 student from Pakistan on a US Government funded program and got the waiver.
