J-1 visa different from Ds-2019

Hi,
My J-1 visa says " Bearer is subject to 212(e). Two year rule does apply."

However, on my DS-2019 form , NONE of the following options have been tick marked.

" Not subject to the two-year residence permit."
" Subject to the two-year residence permit"


So am I subjected to the two-year rule or not?

Whom can I contact to figure this out?
Please let me know soon so I can figure out if I need to apply for a waiver.
