Nori (India) Bio-Data/Affidavit

I am trying to apply for J-1 waiver from within India. This is the most confusing stuff as most of the details online are for applying from within US. I found the Bio-Data and Affidavit forms from https://www.mea.gov.in/nori.htm . This looks like a scanned copy of San Francisco Indian Counslate Nori forms. But if I look at Indian Embassy in Us website, the form is different - https://www.indianembassyusa.gov.in/pdf/Form_Pdf_66.pdf
I dont know which form to use to notarize within India and send to all 3 places for NOC Letter. Also, anyone has experience with http://nori.ac.in/ ?
