Work Permit Visa J2

I am a J2 Visa holder as a "husband" and I know I need an EAD (employment Aythorization Document) in order to work full time. I read the web page carefully but I have a few questions that I would really appreciate if you can help me:

1) The EAD form is I-765. But in the page there are some more that I don't know if I have to fill them, as well. These are: I-485, I-140, DS-2019, IAP-66 and I-94. Are all of them mandatory? Or which one should I or my wife complete?

2) I am planning to work in New York. However, as I have a background in the petroleum industry, I will also want to look something in Houston. So, shall I make 2 EAD - one for NY and the other one for Houston, Texas - or just one EAD, considering that in your webpage Texas and New York should be both deliver to Dallas?

3) I was told I have to start this work permit in United States personally. However, my understanding from the web page is that I can do it by mail. Is that correct? Because it will be wonderful to do it by mail in order not to be 3-4 months in US without working.

Only I-765 to be filed by the J2 holder.

Just 1 form to the appropriate service center.

You can file it by mail or it may be electronically filed. Check USCIS website.

Can i apply for my EAD from India ( after getting J2 visa stamping)

Hi ,
So did you apply for EAD from your home country (by mail) or from US? Im asking because im in same state now and I'll be in India next month. I'll get my J2 stamping from India but i want to apply for EAD by mail from India itself, Can I do that ?
