  • I-485 AOS - Medical Examination

    I'm in process of gathering required documents for my fiance's I485.

    One of requirements are Medical Examination (I-693). But the instruction states that if it's K1, we can only submit the Vaccination Supplement along with I-485.

    1) Is there a specific form for the vaccination supplement?

    2) My fiance has taken multiple shots while doing the physical exam in her country, can she just bring the documents (translated/certified) and attach a copy along with I-485 as vaccination supplement?

    If anyone has knowledge, could you answer for us? It does not say anything about Form DS-3025.

    The doctor giving the immigrant medical exam here, for a K1 entrant, must complete form I-693 and give to you in a sealed envelope. If he chooses to only update the vaccination portion of the I-693, that's okay, but he will also do a TB skin test and blood test. Just request the "short version" of an immigrant medical exam update for a K1 entrant and show him the vaccination sheet your gal received when having her medical exam done for her visa.

      Vaccinations on DS 3025 valid ?

      Hi RayB, it is said in the I-485 instructions that if you entered as a K1 and have done the medical exam through the fiancé visa within the past year, you only need to submit the vaccination sheet, they don't talk about TB skin test and blood test. Like everyone else, I only have the remaining DS 3025 vaccination sheet out of the sealed envelop given at the customs. I checked and it's the exact same content as the vaccination sheet of I 693 only with a different layout. Is it good enough ?

        My blood test and TB skin test were done during the K1 medical exam. I remember now.
