Lost Approval Notice

My fiance lost the approval notice and I read that the it is possible to request a duplicate with the I-824 form. However, a copy of the original approval notice or a copy of the notice of action (I-797) is required to be submitted with the I-824 form. Can the duplicate be requested without any of these original forms?

This question is very important. Please help. Any help is appreciated. Thank you.

Another question is: How long does it take to get the approval notice?

When you adjust your status to become a permanent resident, however, they ask for the approval notice to indicate that you are eligible to file this form. But maybe, we can just use the form DaytonaFiremedic suggested instead. Does anybody know more about this? Help is appreciated. Thank you.
