K1 fiance-affadavit and police report

I am using a co sponsor -Does he submit I-134 or I-864 and do I still submit one also -if so which one?Does it look bad to have a co sponsor?
Does the police report have to be within the past month or within the year?
Does it have to be translated along with the birth certificate?
D-157 ?cant find this form -is it required?
DS-156 and 157 forms-can I prepare for my fiance and send for him to sign as necessary?Thanks

Our embassy would not accept the police certificate without a translation.
