visa issue and time

i now have my wife here in the states,after a little over a year, i am trying to apply for ajustment of status and green card...my questions are:

is she still consider k-1?
what forms should i file...i see many way to do it and its confusing.
any answer would be grateful.

We are about to do our AOS as well and as I entered here in K1 visa I am still classed as that, nothing has changed till AOS is done and approved.
All the instructions for AOS are on this site and on USCIS site. Download them and follow carefully.

Copy of Birth certifcate
Copy of K1 visa in passport
2 passport photos US passport size A number and name on back
Copy of DS3025-- Vaccination report
G325-A Biographic Information
I-864- Affadvit of support with supporting documents Copy of federal tax return.W-2

Copy of I-197-- Notice of action, Approval of K1 petition
Copy of Marriage Certificate
Copy of I-94- Arrival/ departure record, front and back.

She will not send in her DS3025 vaccination report. She will show it to a Civil Surgeon so he will know what shots she has already had and what other shots she may need. She will need another full medical exam because you waited too long for her visa exam to still be valid (over a year here already.) The civil surgeon will fill out the I693 medical report which includes a section showing her immunizations. That full report will be sealed in an envelope for you to include with your other application forms.

Had you applied sooner, her previous exam would have been accepted. They have it on file from when she entered the US.
