H4 Visa for minor when one parent in USA

I am in process for applying H4 for my new born. I am currently in USA.

While applying for Indian passport, I had to mail back a attested affidavit that I have no objection for my wife applying for passport.

Do I need some similar affidavit when I send document to US embassy for H4. I intend to use the waiver program and thus will be mailing all the docs.

If I do need an affidavit / notarized doc, can someone share the format for same.
