H1B Cap Exempted Visa

HI,

I filed my Petition for h1b in 2008 , and got an Approval in Oct 2008. my Petition was vlid till July 2011.
So my petition is expired, but during this period, I dint go for stamping nor did I travel to US.

So my question is:
1) As the 6 year term is not over, Can I file a h1b CAP Exempted petition ? or apply for an extension ?
2) If i can file a ca Exempt petition now, How long will It be valid, I mean the 6 year term will be over in 2014, So I have max 1.5 years more.
3) As I never stayed in usa before, After this expires, Can I apply for an extension of Stay in the US or Extension of this h1b visa ?
