Missed entering middle name while filling i-94 at the JFK port of entry

    Hi,

    My full name is Bhanu Krishna Katuri

    First Name : Bhanu
    Middle Name : Krishna
    SurName : Katuri

    Surname in Indian Passport : Katuri
    Given Name in Indian Passport : Bhanu Krishna

    The name printed on my I-797B is, KATURI, BHANU KRISHNA

    Then name printed on my H1-B visa is as follows,
    Surname : KATURI
    Given Name : BHANU KRISHNA

    I entered my surname(Katuri) correctly but I missed filling in my middle name(Krishna) along with my first name(Bhanu) for the First(Given) Name field in i-94 form while entering the US at JFK Airport thinking that only First Name in the Given name should be entered.

    In short, I entered "Bhanu" instead of "Bhanu Krishna" in the First(Given) name field of I-94.

    Does it create any problem while applying for Social Secuirty Card?

    Is there any way to get my full name (Bhanu Krishna Katuri) on social secuity card instead of (Bhanu Katuri) as entered in i-94?

    Hi Bhanu,

    I do not feel this is something major.

    Your all other documents carry your full name. Your Passport, your visa, your Indian Driving License. There is enough documentary evidence with you to apply for Social Security with the correct name.

    Nothing major, i feel.

        Middle Name missing in i94

        Hi,


        I have the same problem. Can anybody advise my SSN will come through or I need to make some corrections now before SSN comes or after SSN comes ?

        Today I went to the SSN office and the SSA Officer told me that my first(given) name is different from the Visa and Passport compared to i94. She told me that the final decision would be taken by the Department of Home Land Security. If they approve , I will get my SSN.

        My Case is mentioned below.

        Actual :
        First Name : Deepak
        Middle Name : Kumar
        Last Name : Dash

        In VISA & Passport :
        Given Name : Deepak Kumar
        Last Name : Dash


        In i94 :
        First Name : Deepak
        Last Name : Dash

        In SSN Application:
        First Name : Deepak
        Last Name : Dash


        Do I need to correct my i94 now itself or I need to wait till I get some notice from DHS (Department of Home Land Security) ?

        Also please let me know if an incorrect i94 will create nay problems later ? Any help from somebody who has gone through this scenario before.

            Please can you advice me what I should do next.

            My case is almost similar but the issue is while applying SSN i did the following
            My full name is KAPIL KUMAR BAJPAI

            Surname in Indian Passport : BAJPAI
            Given Name in Indian Passport : KAPIL KUMAR

            The name printed on my I-797B is, BAJPAI, KAPIL KUMAR

            Then name printed on my H1-B visa is as follows,
            Surname : BAJPAI
            Given Name : KAPIL KUMAR


            In SSN Application I wrote in application form while applying:

            First Name : KAPIL
            Middle Name : KUMAR
            SurName : BAJPAI


            While applying i was given a letter with information that there is a name mismatch , and it will take little longer to get this resolve .
            Last week I applied and right now i am not sure what i should do next.

            Please advice , appreciate your help.
