Name change for H1B visa renewal

Hi,

I am applying for H1 visa renewal. I renewed my passport in US where I did a name split. Should this be considered as change in name while filling DS-160 form (for question: Have you ever used other names?)

My Old passport had

Given Name - X Y Z
Surname - Blank

My New passport has

Given Name - X Y
Surname - Z

I tried selecting Yes for "Have you ever used other names?" and gave "Other Given Name" as X Y Z and left "Other Surname" Blank, but its telling Surname is mandatory

Please guide me on how to proceed.
