US contact information for H4 VISA-DS 160

Hi..

Plz help me to fill DS 160 Form for H4 Visa

I got H1B Visa and now my wife will apply for H4 Visa..we will travel together to USA.

SO in US Contact information..what should be mentioned??? my name or my Employer name??

if employer then in relation should i select OTHER??

and for address should i mention hotel address (i mentioned hotel address in my DS-160)or company address??

Plz help me on this.

H4 US contact will generally be H1, and address would be H1's address, in your case would be the hotel address.

Thanks 1 more question ...for question "do you have any immediate relative in USA"...we should select yes or No??? as i am(H1) still in india.
