Change name Passport and Visa

My wife's passport has her old name (i.e one before marriage).
She has H1 B visa. Her current visa is stamped with old name . Also she has approval notice for new visa in old name
How do you we change her name in passport and visa ?
If we change name in passport , would that cause issues with visa?
