h1 visa stamping

my green card priority date is Sept 2009 EB 2 category. However I switched company so right now my Labor with the new company is in process. my h1b has already been approved with the new company and valid till June 2016. my last visa stamping was Sept 2008. My wife is already gone to India. I am leaving on June 26th and coming back July 24th.

I am trying to take an appointment for visa stamping on July 5th. Do I need to fill separate DS-160 form for my wife? Will I be okay as far as visa granting? what else should I be careful about?

I will be applying from Kolkata Consulate.

Yes each person has to have their own DS-160.
