Thanks for posting your sample DS-156's on your site. Very useful.
I had some specific questions.

I work for a company whose corporate office is in California and has many satellite offices(I work at one of the offices in Georgia)
My I-797A has the company name and the corporate office address in California.

Question 20: Name and Address of Present Employer
Should the address be the Corporate Office in california or the address that I work at in Georgia.

Question 32:
Do you intend to work in the US: If yes, give name and address.
Same. Should this be the corporate office or the local branch that I work at.
