What is the timeframe for DS -160 form submission and taking appointment for stamping

Hi All,

My H1B petition is approved (for duration Oct-14 to 2017) and now I have to submit DS 160 form. Have few doubts as follows:
1. What is the time frame or last date for this years DS160 form submission?
2. Do I need to submit seperate DS160 form for my spouse(for a dependent visa)?
