When does a H-1B transfer petition becomes a bridge petition:

Can someone explain me for below cases, when does a H-1B transfer petition falls into the category of bridge petition.

My VISA for my current employer A is valid till Sept 30, 2014 (same for I-94 expiry as well), company A has filed my H-1B extension under regular processing on August 28 and I have the EAC number for that as well. Now

i.) company B files my H-1B transfer petition in premium category on Sept 30 or before and the EAC number is generated on Sept 30, is this a case of bridge petition?

ii.) company B files my H-1B transfer petition in premium category on Sept 30, but the EAC number is generated on Oct 1, is this a case of bridge petition?

iii.) company B files my H-1B transfer petition in premium category on Sept 30, but the EAC number is generated on Oct 2 or beyond, is this a case of bridge petition?

I believe as long as I have the receipt number H-1B transfer case before my I-94 expiry, there is no bridge. Or does my transfer petition needs to be approved before current H-1B expiry in order not to fall into bridge category?
