Urgent: Visa Interview Waiver Question

My H1B visa (stamp) expired on 28th May 2014. I had another extension up to July 2014 (6 years completed). I had to move out of US (at London) for 3 months until i become eligible for 7th year extension (AC21/based on perm pending). I'm working for the same employer.

While scheduling an appointment at London embassy, it asks a question - "Is the applicant renewing a Visa?" - Should this be yes in my case?
If I answer yes to this question, it asks me for the issue and expiration date for the visa I'm renewing. - Should this be for the last stamp I had on my passport ? Since I had an extension after my visa stamp expired. Am I still eligible for Visa Interview Waiver?

Yes. it's a renewal
Yes..Last stamp on ur passport that expires in July.
