My husband got his H1 stamped last month end. I have my appointment this month end. My husband is still in India and I would be carrying his passport along to the consulate.

1. Do I need to carry an affidavit stating that my spouse would support me?
2. After marriage, my surname has changed. But my passport reflects my maiden surname. Under Item 8(Other surnames used), should I mention the surname I use after marriage?
3. Item 26(How long do you intend to stay in the US)? What should be the appropriate answer?
