Change of status from H1 to H4

Please help me in the following issue,

My wife came to US on H4 and in the month of November her H1 got approved.

She received SSN about 15 days back. Now her health is not supporting, so I want to apply change of status for her.

She don't have pay slips for COS, since she didn't start her job.

I. Is it necessary to apply for COS, since she didn't start her job.

II. If yes, how to apply for COS with out pay stubs.

Please help, it's urgent.

If her H1 was approved as COS from H4 in November, does it mean she was not working/getting pad till now? If she was not, then she has been out of status since November and it would be difficult to get a COS from H1 back to H4. She would need to leave US and re enter on H4 visa to get back into status.

But she got her SSN in first week of this month.

If she went back to India, is it a problem during H4 stamping?
