i94 update after trasfer and extension of H1B

Hi,
My H1 B visa was transferred from Company A to Company B in July 2014 and Company B filed for a 2 year extension as well however, when I checked my I-94 it still shows the date of my old Visa. How do I get my I-94(date is still valid) to reflect the correct date ? Is there a form that I need to fill out for this ? Do I have to get re-stamped before the old end date on my I-94 ?

Was the extension approved? Did you receive a new I797? What is the date on the new I-94, at the bottom of the I797?
