Need Help Regarding Appointment

Dear All,

I am working on H1 for last 2 years. Now I am going to apply for visa revalidation in India. Today I was trying to book appointment and I came cross some doubt which are

1. A resident of India/Bhutan or An Indian citizen resident in the US (I think option 2nd is correct)
2. I don’t have Surname in passport only given name is there for example “Suresh Kumar “

at time of booking appointment got instruction for Form DS-156 stating that " If you prefer to use only given name and don't use surname enter entire given name in Q6. and FNU in Q7," Since I don't have Surname so I think there is no preference for me. So should I fill NA in Surname(Q6) and Suresh Kumar in First and middle name (Q7) or do I fill Suresh Kumar in Surname column and FNU in First and middle name column.


Please advice me. Thanks a lot in advance.

Thanks a lot. Again wants to clarify first question. Should I select second option " Indian citizen reside in USA" ?
