Start date under Petitioner while scheduling appointment

Hi,
I have new petition valid from 02/04/2016 and current petition is valid till 02/03/2016 and I am going to india and will go for stamping and I think I need to enter New petition number but what details I need to enter below details

Petitioner's Name: Employer Name
Start Date: ? is it my old petition validity start date or new petition validity start date or my joining date with employer?
Receipt Number: new petition number
Expiration Date: new petition expiry date
