Concurrent Filing of H1B extension, H4 extension and H4 EAD renewal

Hi all,

My husband's employer has agreed to include I-765 for H4 EAD renewal along with his H1B and my H4 extension petitions in premium processing. We are in a real tight time crunch. our visas and my EAD expire on June 30th and my new job starts on jUly 1st. I have some queries regarding the I765 application and the cover letter. Please help me out if any of you has gone through the same process.

1) I765 Qs:10- what should I fill in? my current I-94 number, which is about to expire in june end or should I leave it blank?
2) I-765 Qs:17- Should I leave it blank or write my husband's current I797 receipt number, which is about to expire?
3) Should I include my appointment letter in the package as well

Guys please help. Especially question number 18. I mistakenly wrote 17 earlier. Should I write the older I797 receipt number?

1) Yes, you should put your I-94
2) Yes, you should put your husband's most recent WAC number i.e. from I797 receipt
3) Don't know
