H1B Extension & Change In Location At same Time

I am currently in India and my H1B will expire on 23rd August 2016. I have to immediately travel to US for a new client and at new location and hence I need a new LCA and amendment to my H1B. But at the same time I also need to extend by visa beyond 23rd August. I have the following questions

1. How long will take for LCA/Amendment under premium processing and also how long will it take for H1B extension under premium processing?
2. Can I apply for H1B extension and change in location in the same request to USCIS? That way, I won’t have to submit 2 separate request?
3. Is it recommended to first apply for LCA/amendment and then travel to US(New Location) and from US apply for H1B extension?
