H1 extension and Transfer at same time

Hi,

I am working for employer A valid i-94 till Aug 2016 and applied for extension for H1/H4(spouse) in normal processing. I got offer from employer B applied for H1 transfer/ H4(spouse) extension in premium and both approved with i-94 till 2019.

I am planning to join employer B soon.

My question:
1. Is employer A required to cancel the extension petitions. Do they get fee back in case cancel extension petition.
2. If employer A do not cancel extension, what will happen if H1 and H4 gets approved.
3. Which i-94 (employer A or B) shall be applicable for H4.
4. How H1 and H4 i-94 are linked.

Employee A is required to withdrawn their petition should you leave the company however this rarely happens. They will not get any fees back at all. the H4 I do not understand. H4 is for dependents and if you are the primary beneficiary of an H1B petition, why do you have an H4. If you are married, you should apply for an H1B from employer B, just in case the extension with Employer A is withdrawn.
