Dear All,

I have some doubt regarding form DS157. In my passport I have only Given Name " Suresh Kumar" and there is no Surname. So as per instruction I filled in DS156 Complete given name "Suresh Kumar" in Surname column(Q6) and FNU in First name (Q7) and when I start filling DS157, Q1(last name) was autofilled with Suresh Kumar and Q2.(First Name) was FNU and Q3. was Blank. My question is this do I need to modify DS157 as Last name " Kumar" an Q2(First Name) Suresh or should I leave as is it.

I also wrote to VFS regarding this query but they did not give proper answer.

Please advice me. Thanks in advance.

Just make sure that you have entered your name as it is on your passport on the DS 156. You can allow the name to be duplicated on the DS 157 the way it appears. In column 3 you will have to write your name in native alphabets after you take a print-out.
