H1B Extn Premium Pending more than 15 days

I have filed H1B Extension + COS H1b to H4 (Spouse) + H4 EAD at VSC in Premium processing.

Primary Applicant:
My current H1B expires on 12/31/2016.

Dependent Applicant:
Currently on H1B and expires on 09/19/2016.

H1B Extension Received Date: 07/27
Spouse COS H1B to H4 Received date: 07/26
Spouse H4 EAD Received Date: 07/26.

Spouse COS H1B to H4 & H4 EAD Approval Date: 08/09. (Aug 9th)
H1B Extension Still Pending.

1) Its strange that H1B Extn is till pending while dependent applications have been approved.
2) Received approval notice for Spouse H4EAD but the start date is NOT the approval date instead its from 01/01/2017 (future date as per Primary applicant). Now, Dependent applicant cannot work until 01/01/2017.Can you please advise?

Also, I am still waiting for approval notice for COS (H1B to H4) in order to see the validity dates.

We have raised a service request to USCIS to correct the dates for H4EAD and have received new H4EAD.

What was the issue with your H1 Extn on Premium processing, why did that delay? Mine is almost 20 days now and the case status still the same "Case Was Received and A Receipt Notice Was Emailed".

I got receipt on 09/13/16 for H1B extension in premium processing, but still I didn't get any update on processing. Did you guys got your H1B extension approval, If yes how many days it took? I have already planned an INDIA trip and waiting for this.

Center is: VSC (Vermont Service Center)

In my case, first H1 to H4 and H4EAD were approved before H1B extension and then attorney has raised a service request after 15 calendar days. I have received H1B extension approval after 3 days from the day the service request was raised.

I think it's taking ateast 6-7 months.
