Ds -156 form filing - questions

Hi,
I have 2 questions regarding filling up form 156.

1. When do you intend to arrive in US?(Provide specific date if known)

I don't have my itinerary so is it alright to skip this question?

2. How long to do intend to stay in US?

What should be specified here if you applying for H1? 3 years?
I hope this number of years is independent of the previous stays i was in US on L1.
(on some places on counsleate web site it states never leave any column blank say 'NIL' or 'NONE'. So do you think I should say 'Nil' for the first question too?)

It doesn't matter. If you know a specific date on which You are planning for your arrival to US, just specify the date. There is no harmness.
Also the duration of stay depends on what your consultancy said to you. Again these are all just formal questions and not really mind boggling questions.
