I am going to india and while coming back i need to get my visa stamped, By the way i have few questions with respect to application DS-156,

Question(15) Home Address -> is this India address or US address

Question(26) How Long Do You Intend To Stay in The U.S -> I got 7th year extension and its going to expire in June , So should i say 1 Year >

Question (30) Have You Ever Been Issued a U.S. Visa ? - Visa issued in US or ?

Jun'07 (till the validity of Visa)
Question (30) Have You Ever Been Issued a U.S. Visa ? - Visa issued in US or ?
If it was apprroved earlier in US say "Yes, US", you can also put all prev approvals in a separate paper"
