Can anybody help me out?

I have got an H1B approval notice. I am in Gujarat, India. I want to process the Visa issuance for My family [H4 visa] along with my H1B.

1 Is there any other fees for H4 visa processing [excluding VFS Fees and Visa issuance fees]

2 Have I to pay the VFS Fees separately for my child [4 yrs. of age] separately.

3 How can I set appointment for Mumbai consulate altogether for H1B and H4 visa online.

4 Do I need to take my child [4 yrs] for the getting visa interview to Mumbai.

USE "ADD APPLICANT" FEATURE WHILE MAKING THE ONLINE APPOINTMENT.
