Two small Q's in DS-156

I am planning to go to Mexico for Visa revalidation and while filling up DS156, I got confused at couple of questions. They are

Q: 15(in DS156)
Home Address (Include apartment number, street, city, state or province, postal zone and country)
Street Address Line 1

Should I write my Indian address or USA address ?

Q: 36. Has Anyone Ever Filed an Immigrant Visa Petition on Your Behalf?

My previous employer has filed for Labor and then I left the company. I don't even know what happend to that now.

What should be my answer to the above question ?



Q 3: Do I need to print DS156 in one page (both sides) or it can be on two pages ?

---- NO, An immigration petition is only when an employer files I-140 and is approved after the labor clearance. If your previous employer has applied for your labor have you followed up if I-140 is applied

---- Print on one page (both sides)
