How can i fill D157, if it is not there

Hi All,

I cancelled my previous appoint ment and rebooked for new appointment date
visa interview. But i was able to fill D156 only. I did not find D157 to fill.
Is it system mistake or is it not necessary to fill D157 second time.
Why i did not find the D157 to fill. But still i retrieving the files. I was able to see D156 only.
Any body faced like this problem , please share ur experiences.
