Name Misspelt on H1 Approval - Correction Process

HI,

I would like to know the name correction process. My H1 was approved and issued with error in spelling my name. I would appreciate if anyone can guide me in process of applying for correction in name.

Like what are the forms?
what documents are to be enclosed?
What fee has to be sent to INS?
Which service center it has to be sent?
