D156 & D157 Forms filling

Hi All,

I have applied miselaneous services like change of address and inclusion of spouce name for my wife and mine. They have issued me a new passport by including all the above said services for both of us.

While filling the D156 & D157 forms to obtain visa appointment which passport details ( like the Passport Number, Issuance Date (dd-month-yyyy), Expiration Date (dd-month-yyyy) etc.,)
I need to furnish for both of us.

Could anyone please shed some light on this.
