I would like to know what I can fill in the DS-156 in the following columns.

15 Home address- i live in US and going to India for H1B stamping. Should this be india or US address. Its too cofusing, there are conflicting answers.

36 Has anyone filed a immigrant visa petition?
If yes, should it be the person's name or the company's name?
