OS156 Or DS 156 confusion

Hi, I want to revalidate my visa and I visited revalidate webpage of this site for info. It says that, we need to fill DS156 form, but, my friend, who recently revalidated her visa says, she sent OS156, can anybody please clarify this confusion, on which form to send for H1 visa revalidation ?
