Affidavit - Help Needed

There is some name conflict on my passport and on petition. So can I make an affidavit saying these two names belong to same person? Is this work at Stamping?
Also is there any format for Affidavit which will work in US? I am going to make this affidavit in India.

even i have the same problem due to my long name last 3 letters of my word were eliminated...

i came to know from employer and other resources that its not a problem and it wont cause a problem ...but better confirm with ur employer too.


Neither an amended nor a new petition is required to be filed if the petitioner merely changes her/his name, provided all other the terms and conditions of the alien's employment remain the same. The petitioner, however, has the obligation to notify the INS of the new name when it files for an extension of the H1B Petition.

Request you to go through the link and ask ur employers and then decide..
