H4 to H1 COS, Spouse details needed?

Hi,

My wife is on h4 visa & her h1b for 2008 is approved.
She entered USA in June 08 on h4 visa.
In July 08, I switched my company(employer) from A to B & filed for H1b x'fer which is still pending.
I joined the company B in Sept, even though the transfer is still pending. Note that my wife's h4 transfer is not filed after my switch to new company-B.

Now my wife's employer has a project on the urgent basis & want to file her COS from h4 to h1b under premium processing.
1. To file that COS, do that employer need my pay-stubs & my petition papers? ( In fact , her employer asked for these docs. ) Which petition papers should I give to my wife's employer - a)petition docs on company-A name (note-My latest pay stubs are on the company-B name.) or b)the H1B transfer receipt on the company-B name.
2. Right now my h1b xfer is pending & her h4 x'fer is not filed, then will it affect the wife's COS processing?
3. If her COS is rejected, does that mean her h1b is rejected ? Can she go for h1b stamping in India/Mexico/Canada ?
