H4 visa. Delay in getting passport

I appeared for my Non-immigration H4 visa interview on 2nd December,2008 in Delhi Consulate. The consular accepted my passport and told I will get my Visa within 7 days.
Its been 14 days and VFS is yet to receive my passport from US Consulate.

Has anyone faced a similar situation.
Any idea what could be the reason ?

May be a name check delay if you have a common last/first name.

This is the response I got when I e-mailed Delhi Consulate

"Your visa application is currently on hold under Section 221(g) of the Immigration and Nationality Act (INA) as the electronic record of your petition has not yet been received."

Whose petition docs are they waiting i.e. H1 docs or my H4 docs ?
Have they lost the petition docs ?
Anyone knows how much time it take for such a case to get the Visa ?
