ITIN needed.

hi,

i am on l1 and my wife is on l2 dependent visa. I tried to get her SSN but there was some data discrepancy. now it is time to file tax returns.

1) is my wife eligible to apply for her itin now?
2) if so, do i need to apply a separate form now or apply along with completed tax return
3) what all documents are needed.

i am in texas. please reply
