H4 Visa Petition Clarification

I need some clarifications... My wife is planning to attend for H4 restamping as she went to India and our H1 and H4 stampings expired last month, we both got our new I797s.. mine is H1B and hers is on H4...

We are trying to fill out the application through VFS website to take an appointment...
In VFS website there is a section to be filled in
"Petition Details", I am not sure what to fill out for the following questions:
1) Principal Applicants Name?
2) Petition #?? mine or hers??
3) Petition expiry date?...

Please advice.. your reply is appreciated..
