Cost of Visa Transfer

Hi,

I have a friend who is currently entertaining several offers for internships in the United States. One is an H2B offer, another a J1 offer, but he is looking to work in the US on an H1B status. If he receives the H2B visa or J1 visa and wants to at some point transfer to H1B status, what associated costs would he have to pay to do so. I know costs will vary regarding several factors, but he would like to know if it would be in the $100's or $1000's or more to do. Also, how long can these visas be extended. For instance, his H2B visa is issued for 6 months and he wishes to file an extention. How many times and how often can he file?
